I would like to retain Kre8tive Law and confirm that I have reviewed the Terms and Conditions, Privacy Policy and Online Legal Services Retainer Agreement posted on this website and agree to be bound by the terms of those documents.

Individual

If I am an individual, I promise to provide Kre8tive Law with a copy of an original government issued identification that is valid and has not expired, including a driver's licence, birth certificate, provincial or territorial health card (if such use of the card is not prohibited by the applicable provincial or territorial law), passport or similar record within 14 days of electronic submission of this form.

Corporation or Society

If I am a corporation, society or other organization created or registered pursuant to legislative authority, I promise to provide Kre8tive Law with a copy of a written confirmation from a government registry as to the existence, name and address of the organization and the name's of the organization's directors, such as:

1. a certificate of corporate status;
2. a copy of an annual return;
3. a copy of a similar record obtained from a government registry that confirms the organization's existence,

within 60 days of electronic submission of this form.

Trusts and Partnerships

If I am an organization other than a corporation or society, I promise to provide Kre8tive Law with a copy of the documents creating the organization, such as a trust or partnership agreement or other record confirming the existence of the organization within 14 days of electronic submission of this form.

Copies of the above-noted documentation may be delivered by fax, email or regular mail to Kre8tive Law and I acknowledge that Kre8tive Law has the right to examine the original of the submitted identification.
