The Estate Planning Worksheet Form and  Other Forms are in MS Word, and MS Excel to make them easy for you to download onto your computer and save.  The Estate Planning Worksheet Form should be downloaded, completed and faxed back to 1-888-613-1497. The other forms have been formatted so that you can type data directly into the boxes on a particular form. Once you have completed the indicated form and saved it on your computer, simply email or fax it back to us.  If you have any problems with any of the forms or getting them to download to your computer, please call and one of our staff will assist you.

Please take the time to complete either the Estate Planning Worksheet Form or other forms and return it to us, PRIOR to setting an appointment.  This will allow our initial conference to be most productive.
